Product Form

The product review header displays the title of the product currently being reviewed. It also contains a percentage completion meter at the top of the header. The completion meter does not progress until a consumer moves on to the next form section or product. Any valid and applicable product image will be displayed with the relevant product on the feedback form if a product section is included in the campaign.

  • The product Star rating question is mandatory if included. It will display a five-point star rating along with a customisable title and subtitle.

  • The product Main review question can be optional. It includes a customisable title, placeholder copy, and hint text. The product main review can also have a maximum number of characters set, with no minimum.

  • The Product attribute (fit) question can be optional. It includes a customisable title and subtitle. It provides default values of too small, fits perfectly, and too big.

  • The Product attribute (5-point scale) question can be optional. It includes a customisable title and subtitle. It provides a default 5-point scale, displayed as graphical emotions. These translate to our usual 5-point rating scale for product attributes.

  • The Media upload question can be optional. It includes a customisable title and subtitle. The consumer can upload media, image or video, to the review. The image will display within the form once it has uploaded successfully, and can be removed by the consumer if desired.

     

  • The Demographics question can be optional. It includes a customisable title and subtitle. It provides the consumer with options to choose from. 

  • The Custom input field question can be optional. It includes a customisable title, placeholder copy, and hint. The custom input field can also have a maximum number of characters set, with no minimum.

  • The Custom text box question can be optional. It includes a customisable title, placeholder copy, and hint. The custom text box can also have a maximum number of characters set, with no minimum.

     

  • The Custom single select question can be optional. It includes a customisable title and subtitle. It provides the consumer with options to choose from.

     

  • The Custom multi-select question can be optional. It includes a customisable title and subtitle. It provides the consumer with options to choose from of which the maximum selection can be set within the form.

     

  • The Custom rating scale question can be optional. It includes a customisable title and subtitle. It provides a default 5-point scale, displayed as graphical emotions. These translate to our usual 5-point rating scale for product attributes.

     

  • The Section banner question can be optional. It can be displayed anywhere within the feedback form and can act as a banner with a customisable title and subtitle.
